Drawing on a landscape analysis of existing data-sharing initiatives, in-depth interviews with expert stakeholders, and public deliberations with community advisory panels across the U.S., we describe features of the evolving medical information commons. We identify participant-centricity and trustworthiness as the most important features of an MIC and discuss the implications for those seeking to create a sustainable, useful, and widely available collection of linked resources for research and other purposes.

RATIONALE, AIM AND OBJECTIVE: Factors influencing doctors in prescribing of drugs have mostly been studied in primary care. Studies performed in hospital care have primarily focused on new drugs, not prescribing in general. An in-depth understanding of the prescribing process in the more specialized secondary care is not only important for secondary care itself, but because it also influences prescribing in primary care. Much of children's early syntactic development can be seen as the acquisition of sentence-level constructions that correspond to relatively complex events and states of affairs. The current study was an attempt to determine the relative concreteness (verb-specificity) or abstractness (verb-generality) of such constructions for children just beginning to produce large numbers of multi-word utterances. Sixteen children at 2.0 years of age and sixteen children at 2,5 years of age participated (all English speaking). Each child was taught two novel verbs for (...) a highly transitive action: one in a transitive construction (Ernie is tamming the car) and one in an intransitive construction (with patient äs subject: The ball is meeking). They were then given opportunities to use their newly learned verbs, in many cases in discourse situations that encouraged use of the "opposite" construction (i.e., agent and patient-focused questions). Results showed that 2.0-year-old children almost never produced an utterance using a novel verb in anything other than the construction in which it had been modeled. Children at 2.5 years of age were somewhat more productive, but still the large majority of these children avoided using the experimental verbs in nonmodeled constructions. These results suggest that when English-speaking children produce simple transitive and intransitive utterances in their spontaneous speech, they are doing so on a verb-specific basis (verb Island constructions), schematizing more abstract constructions only later äs they discover patterns that apply across many such lexically specific constructions. Meaningful participant engagement has been identified as a key contributor to the success of efforts to share data via a “Medical Information Commons”. We present findings from expert stakeholder interviews aimed at understanding barriers to engagement and the appropriate role of MIC participants. Although most interviewees supported engagement, they distinguished between individual versus collective forms. They also noted challenges including representation and perceived inefficiency, prompting reflection on political aspects of engagement and efficiency concerns.

To revitalize nursing science, there is a need for a new approach to guide nurse scientists in addressing complex problems in health care. By applying theoretical concepts from a revolutionary philosopher of science, Paul K. Feyerabend, new nursing knowledge can be produced using creativity and pluralistic approaches. Feyerabend proposed that methods within and outside of science can produce knowledge. Despite the recognition of Feyerabendian philosophy within science, there is currently a lack of literature regarding the relevance of Feyerabendian philosophy for (...) nursing science. We aim to (a) describe and critique Feyerabendian concepts, (b) discuss the potential application of Feyerabendian philosophy for knowledge production within gerontological nursing and (c) describe theoretical possibilities for nurse scientists in using Feyerabendian philosophy to guide nursing knowledge development. We begin by introducing Feyerabend's life and his inspirations for his theoretical concepts, epistemological anarchism, theoretical pluralism and humanitarianism, and conclude by offering suggestions of how to apply Feyerabendian philosophy in nursing research. In this book, Michael Arbib, a researcher in artificial intelligence and brain theory, joins forces with Mary Hesse, a philosopher of science, to present an integrated account of how humans 'construct' reality through interaction with the social and physical world around them. The book is a major expansion of the Gifford Lectures delivered by the authors at the University of Edinburgh in the autumn of 1983. Radical political thought of the 20th century was dominated by utopia, but the failure of communism in Eastern Europe and its disavowal in China has brought on the need for a new model of utopian thought. This book thus seeks to redefine the concept of utopia and bring it to bear on today's politics. The original essays, contributed by key thinkers such as Gianni Vattimo and Jean-Luc Nancy, highlight the connection between utopian theory and practice. The book reassesses the legacy (...) of utopia and conceptualizes alternatives to the neo-liberal, technocratic regimes prevalent in today's world. According to the evolutionary sceptic, the fact that our cognitive faculties evolved radically undermines their reliability. A number of evolutionary epistemologists have sought to refute this kind of scepticism. This paper accepts the success of these attempts, yet argues that refuting the evolutionary sceptic is not enough to put any particular domain of beliefs – notably scientific beliefs, which include belief in Darwinian evolution – on a firm footing. The paper thus sets out to contribute to this positive justificatory project, (...) underdeveloped in the literature. In contrast to a ‘wholesale’ approach, attempting to secure justification for all of our beliefs on the grounds that our belief-forming mechanisms evolved to track truth, we propose a ‘piecemeal’ approach of assessing the reliability of particular belief-forming mechanisms in particular domains. This stands in contrast to the more familiar attempt to transfer warrant obtained for one domain to another by showing how one is somehow an extension of the other. We offer a naturalist reply to the charge of circularity by appealing to reliabilist work on the problem of induction, notably Peter Lipton's distinction between self-certifying and non-self-certifying inductive arguments. We show how, for scientific beliefs, a non-self-certifying argument might be made for the reliability of our cognitive faculties in that domain. We call this strategy Humean Bootstrapping. (shrink)

A medical information commons is a networked data environment utilized for research and clinical applications. At three deliberations across the U.S., we engaged 75 adults in two-day facilitated discussions on the ethical and social issues inherent to sharing data with an MIC. Deliberants made recommendations regarding opt-in consent, transparent data policies, public representation on MIC governing boards, and strict data security and privacy protection. Community engagement is critical to earning the public's trust.

Bioethics, clinical ethics, and professional ethics are mature, well-developed fields of applied ethics that focus on medical research, patient autonomy and patient care, patient–healthcare professional relationships, and issues that arise in clinical and other medical settings. However, despite these developments, little attention has been paid to the organizational aspects of healthcare in these fields. This is surprising, because in the last 30 years healthcare has become more and more institutionalized in provider, management, and insurer organizations. Despite JCAHO's preoccupation with organizational (...) ethics during the last decade, the philosophical underpinnings of their requirements have been less explored in the literature. Clinical ethics remains preoccupied with clinical patient care and professional ethics with individual professional guidelines; even the American College of Healthcare Executives focuses primarily on healthcare managers, not on healthcare organizations. The fact of evolution raises important questions for the position of moral realism, because the origin of our moral dispositions in a contingent evolutionary process is on the face of it incompatible with the view that our moral beliefs track independent moral truths. Moreover, this meta-ethical worry seems to undermine the normative justification of our moral norms and beliefs. If we don’t have any grounds to believe that the source of our moral beliefs has any ontological authority, how can our (...) moral judgments be justified in an objective way? In this chapter, I argue that while traditional moral realism is untenable in the light of evolution, normative justification should not be handed the same fate. It is precisely in the fact that moral norms and beliefs are grounded in evolved, innate and therefore universally shared intuitions that those norms and beliefs can be objective-for-us. Such an internalist justification allows us to differentiate moral right from wrong, not because some feature of the external world forces us to acknowledge this, but because our moral nature forces us to project this moral judgment on the world. What’s more, guided by this innate moral compass we can both assess and realize moral progress. (shrink)

To test if dreams contain remote or never-experienced motor skills, we collected during 6 weeks dream reports from 15 paraplegics and 15 healthy subjects. In 9/10 subjects with spinal cord injury and in 5/5 with congenital paraplegia, voluntary leg movements were reported during dream, including feelings of walking , running , dancing , standing up , bicycling , and practicing sports . Paraplegia patients experienced walking dreams just as often as controls . There was no correlation between the frequency of (...) walking dreams and the duration of paraplegia. In contrast, patients were rarely paraplegic in dreams. Subjects who had never walked or stopped walking 4–64 years prior to this study still experience walking in their dreams, suggesting that a cerebral walking program, either genetic or more probably developed via mirror neurons is reactivated during sleep. The debate about scientific realism is concerned with the relation between our scientific theories and the world. Scientific realists argue that our best theories or components of those theories correspond to the world. Anti-realists deny such a correspondence. Traditionally, this central issue in the philosophy of science has been approached by focusing on the theories themselves (e.g., by looking at theory change or the underlying experimental context). I propose a relatively unexplored way to approach this old debate. In addition to (...) focusing on the theory, we should focus on the theorizer. More precisely, in order to determine on which component of a theory we should hinge a realist commitment, we should analyze the cognitive processes underlying scientific theorizing. In this paper I do just that. Drawing from recent developments in the cognitive sciences and evolutionary epistemology, I formulate some tentative conclusions. The aim of this paper is not so much to defend a particular position in the debate on scientific realism but to showcase the value of taking a cognitive perspective in the debate. (shrink)

Empirical evidence from developmental psychology and anthropology points out that the human mind is predisposed to conceptualize the world in particular, species-specific ways. These cognitive predispositions lead to universal human commonsense views, often referred to as folk theories. Nevertheless, humans can transgress these views – i.e. they can contradict them with alternative descriptions, they perceive as more accurate – as exemplified in modern sciences. In this paper, I enquire about the cognitive faculties underlying such transgressions. I claim that there are (...) three faculties enabling us to part with these universal commonsense views of the world imposed by our nature. The first is our ability to represent representations – i.e. to form metarepresentations. The second is our ability to produce alternative representations both by explaining a familiar subject matter in terms of the principles governing different conceptual domains than the one that we are predisposed to apply to the subject matter and by directing our mind to new subject matters (for which we have no predisposed conceptual grasp), understanding them in terms of familiar domains. The third, finally, is our ability to give these representations an epistemic orientation. In the dissertation I provide an account of action descriptions which emphasizes their role as explanations of consequences. By showing that consequences are ascribed to an action under a description, and only when that description can explain the consequence, I undermine the view that consequences are brute events. Roughly, I reason as follows. If consequences were brute events, then their ascription to an action wouldn't hinge on how we understand the action. We could, for instance, say in ordinary circumstances "John (...) tensed his finger" and as a consequence "Mary became a widow" without any untowardness at all. I show both that we do not do this and that we cannot do it. That we do not do it is supported primarily by linguistic intuitions; mainly I show that there is an infelicity in ascribing to an action a consequence which is not explained by that action. To support the claim that we cannot do this I argue that if there were no "fit" between action and consequence that would make communication difficult. ;I then use this characterization of action descriptions and consequences to serve as a criterion for identifying the privileged description of an action. Any one action may have several action descriptions. In light of this, there is a question raised about which, if any, of these descriptions is privileged. I show that within the wider social context there is a description of the action which is dominant. Then I argue that this description is and should be chosen in virtue of the explanatory power of action descriptions with respect to consequences. (shrink)

Images That Move is concerned with how images take place in wider worlds: how they move around, via processes of transmission and uptake, but, equally importantly, how they move their audiences affectively. Images play a significant part in projects of "poetic world-making" and political transformation. They participate in the production of commensuration or of incommensurability, enact moments of prophecy or exposure, and attract or repel spectators' attention. Images move, then, but not just as they wish, and any examination of images (...) in motion must also recognise the blockages and breakdowns that prevent their movement as well as the enframings or "stickinesses" that trap them in particular places, prevent them from reaching others, or hedge their arrival in specific ways. The contributors explore topics ranging from high art to mass media, religious iconography to pornography, and popular photography to political cartoons in a range of contexts and media including photography in early twentieth century China, art and literature in contemporary South Africa, upscale real estate development in India, occult media images and the aesthetic of appearance in urban Indonesia, and film censorship in Nigeria. All of these instances of the visual continuum require different ways of highlighting and addressing the problematics of images in motion. Images That Move traces some of the tangled paths of images' travels and returns, unfolding their effects and aftereffects and scanning the diverse and variable publics-fixed or ephemeral, situated or dispersed-that these moving images call into being. In recent work, Atran, Henrich, Norenzayan and colleagues developed an account of religion that reconciles insights from the ‘by-product’ accounts and the adaptive accounts. According to their synthesis, the process of cultural group selection driven by group competition has recruited our proclivity to adopt and spread religious beliefs and engage in religious practices to increase within group solidarity, harmony and cooperation. While their account has much merit, I believe it only tells us half the story of how institutional religions have (...) evolved. Their cultural evolutionary account of religion only looks at the cultural dynamics arising from competition between groups, not at the dynamics arising from within the group. Drawing from game-theoretic analyses of the emergence and cultural evolution of social institutions, I outline two sets of important ‘within-group’ dynamics that shape institutional religions. The first follow from the necessity to keep the interaction of the participants in an equilibrium state in order to maintain the social institution. The second arise from the competition of institutional features for traction within the group. Bringing these dynamics into account enables us to explain prominent features of institutional religions that cannot be satisfactorily explained by the current model of the cultural evolution of religions. Universal social institutions, such as marriage, commons management and property, have emerged independently in radically different cultures. This requires explanation. As Boyer and Petersen point out ‘in a purely localist framework would have to constitute massively improbable coincidences’ . According to Boyer and Petersen, those institutions emerged naturally out of genetically wired behavioural dispositions, such as marriage out of mating strategies and borders out of territorial behaviour. While I agree with Boyer and Petersen that ‘unnatural’ institutions cannot thrive, this one-sided (...) explanation of universal social institutions in terms of genetic human nature is unsatisfactory. Drawing on the literature on multi-level selection and gene-culture coevolution, I argue that universal social institutions are first and foremost the products of cultural selection. They occupy fitness peaks in the landscape of cultural possibilities, much in the same way that biological adaptations occupy fitness peaks in the landscape of biological possibilities. To show this, I use game-theory. By modelling the domains of social interaction in which marriage, commons management, and property emerged as Prisoner’s dilemma situations, it becomes clear how an institutional framework allows the group to move to an interactive equilibrium with a larger payoff. Institutions do so by incentivising all parties to adopt a cooperative strategy. They are culturally selected ways of optimising genetically constrained domains of human social interaction. (shrink)

Reasoning from a naturalistic perspective, viewing the mind as an evolved biological organ with a particular structure and function, a number of influential philosophers and cognitive scientists claim that science is constrained by human nature. How exactly our genetic constitution constrains scientific representations of the world remains unclear. This is problematic for two reasons. Firstly, it often leads to the unwarranted conclusion that we are cognitively closed to certain aspects or properties of the world. Secondly, it stands in the way (...) of a nuanced account of the relationship between our cognitive and perceptual wiring and scientific theory. In response, I propose a typology or classification of the different kinds of biological constraints and their sources on science. Using Boden’s notion of a conceptual space, I distinguish between constraints relating to the ease with which we can reach representations within our conceptual space and constraints causing possible representations to fall outside of our conceptual space. This last kind of constraints does not entail that some aspects or properties of the world cannot be represented by us – as argued by advocates of ‘cognitive closure’ – merely that some ways of representing the world are inaccessible to us. It relates to what Clark and Rescher have framed as ‘the alien scientist hypothesis’. The purpose of this typology is to provide some much needed clarity and structure to the debate about biological constraints on science. (shrink)

According to some philosophers, we are “cognitively closed” to the answers to certain problems. McGinn has taken the next step and offered a list of examples: the mind/body problem, the problem of the self and the problem of free will. There are naturalistic, scientific answers to these problems, he argues, but we cannot reach them because of our cognitive limitations. In this paper, we take issue with McGinn's thesis as the most well-developed and systematic one among the so-called “new mysterians”. (...) McGinn aims to establish a strong, representational notion of cognitive closure: a principled inaccessibility of a true theory of certain properties of the world, but he offers arguments that only bear on difficulties with psychologically grasping the correct answers. The latter we label psychological closure. We argue that representational closure does not follow from psychological closure, and that McGinn's case therefore falters. We could very well be able to represent the correct answer to some question, even without being able to grasp that answer psychologically. McGinn's mistake in deriving representational closure from psychological closure rests on a fallacy of equivocation relating to the concept of ‘understanding’. By making this distinction explicit, we hope to improve our thinking about the limits of science in particular and human knowledge in general. (shrink)

Influential thinkers such as Young, Sugden, Binmore, and Skyrms have developed game-theoretic accounts of the emergence, persistence and evolution of social contracts. Social contracts are sets of commonly understood rules that govern cooperative social interaction within societies. These naturalistic accounts provide us with valuable and important insights into the foundations of human societies. However, current naturalistic theories focus mainly on how social contracts solve coordination problems in which the interests of the individual participants are aligned, not competition problems in which (...) individual interests compete with group interests. In response, I set out to build on those theories and provide a comprehensive naturalistic account of the emergence, persistence and evolution of social contracts. My central claim is that social contracts have culturally evolved to solve cooperation problems, which include both coordination and competition problems. I argue that solutions to coordination problems emerge from “within-group” dynamics, while solutions to competition problems result largely from “between-group” dynamics. (shrink)
